This contract provides gap services by Community Medical Services to individuals who are uninsured or underinsured and cannot afford methadone treatment. These services are provided until the client can secure sustained funding for their treatment. <br><br>This Modification No. 4 replaces the Scope of Services, extends the contract through September 30, 2028, and makes revisions and additions to the contract provisions.<br><br><strong>Contract Information: </strong><br>Contract No.: 4400006261<br>Contractor Name: Community Medical Services<br>Contract Modification Amount: $75,000.00<br>Contract Period: October 1, 2026, through September 30, 2028<br><br><strong>Current Contract Expenditures: </strong>Within the last 12 months, $75,000.00 has been spent against this contract. <br><br><strong>Previous Contract and Modification Activity:</strong><br>On December 2, 2025, the Commissioners Court approved Modification No. 3, in the amount of $75,000.00, and extended the contract through September 30, 2026.<br><br>On December 12, 2023, the Commissioners Court approved Modification No. 2, which increased the NTE amount from $65,000.00 to $150,000.00 for two renewal terms. <br><br>On April 4, 2023, the Commissioners Court approved Modification No. 1, which increased the NTE amount from $50,000.00 to $65,000.00.<br><br>On March 29, 2022, Commissioners Court approved the initial contract, in the amount of $50,000.00.
