On August 1, 2017, the Purchasing Agent approved this contract for a Revolution EVO CT Scanner for Forensic Scanning. The Medical Examiner's Office is requesting termination of the agreement. A new CT Scanner has been purchased through the Request For Proposal solicitation process. Maintenance and support of the GE CT Scanner is no longer required.<br data-pasted="true"><br>The Medical Examiner's Office requests this contract be terminated in accordance with Section 4 of Modification 5, Amendment to Contract, using a 60-day written notice. The Amendment to Agreement document acknowledges that August 16, 2026, is the termination date. The Service Agreement Addendum document closes out the payment schedule and credits the remaining balance due for the current term of the maintenance and support.<br>  <br><strong>Current Contract Expenditures:</strong> Within the last 12 months, $72,178.72 has been spent against this agreement.
