2025-1405 Authorize and approve Amendment No. 2 to the Texas Department of State Health Services Contract No. HHS001432000003 – Community and Clinical Health Bridge Project to provide $60,000 for the period of September 1, 2025 through August 31, 2026, to support the implementation of obesity prevention strategies that reach high-risk, high-need population On a motion made by Commissioner Dr. Elba Garcia, and seconded by Commissioner Andrew Sommerman, the following order was passed and adopted by the Commissioners Court of Dallas County, State of Texas: BRIEFING DATE: December 2, 2025 FUNDING SOURCE: Fund: 46600 Project: 08908 Be it resolved and ordered that the Dallas County Commissioners Court does hereby approve Amendment No. 2 to the Texas Department of State Health Services Contract No. HHS001432000003 – Community and Clinical Health Bridge Project to provide $60,000 for the period of September 1, 2025 through August 31, 2026, to support the implementation of obesity prevention strategies that reach high-risk, high-need population, and authorize electronic execution of the contract.
